Preschool

Please complete and return to VCA office the following forms and requested items for new enrollment for Preschool:

  

- All forms located in the back of the Preschool

   Handbook (available online at this time by clicking the following link)

      Preschool and Daycare Licensing Forms

- VCA Enrollment packet

- Shot records

- Physician's Report located in the Preschool and Daycare Licensing Forms above.

- Parents Must read the Preschool Handbook Available online, and sign the parent signature page in the handbook.

 

Kindergarten

Please complete and return to VCA office the following forms and requested items for new enrollment in Kindergarten:

- VCA Enrollment packet

- Shot records

- Birth certificate

 

For all other grades please complete and return to VCA the following forms and requested items for new enrollment:

- VCA Enrollment packet

- High School Students transfering in from another high school must  submit a copy of their high school transcript.

- Parents must read the High School Handbook Available online, and sign the parent signature page in the handbook.
