Sport Forms

The following forms are required by Valley Christian Academy to be filled out and returned each school year before a high school student starts the first day of practice of the first sport they're participating in that year.

 CIF Transfer Eligibility Forms

All  high school students tranfering to VCA that are not incoming freshmen that are interested in playing sports at VCA and have not been enrolled at VCA for the past 12 calendar months, CIF paperwork must be completed by the student and parent(s), their previous school and returned to the VCA office. Each student will need to have both the following 207 and the 510 forms completed.

Pre-Enrollment Contact Affidavit
(Must be submitted with Form 207 or a
Foreign Student Request)

If a student makes a valid change of residence (or is a foster student) and changes schools, that student must complete the new Form 206. In this case the 510 form isn't necessary.
